Complete the online application Click here to apply online. Once the form is completed, you will receive an e-mail confirming its submission. Be sure to print your application and have a parent or guardian sign it before mailing it to our office. If you are having issues with the online format, please click here to download the paper application form. |

Complete the student questionnaire After we receive your application, we will send you the Student Questionnaire e-mail to ensure compatibility between student and program expectations. We will contact you if we have any questions or concerns, and to confirm official acceptance into Lifeworks. |

Complete the forms & planning packet In late March, we will send you an information packet with checklists, medical forms, packing lists, summer communications information, travel and contact instructions, etc. Please read through carefully and return the necessary forms to our office. |
